Website privacy notice
This notice explains how information is handled when you use phase.care. Effective September 7, 2026.
Information you choose to provide
If you use the general contact form, Phase Care receives the information you enter, such as your name, email address, optional phone number, and message. We use it to respond to your request and manage practice communications.
The membership enrollment form collects identity and contact details, your selected provider, membership agreement acceptance, optional communication consent, and billing preferences. When you continue to card setup, Phase Care’s enrollment service sends those details to Atlas. Selecting the final enrollment button submits your enrollment for the practice to review.
Stripe collects card numbers, expiration dates, and security codes directly in its secure card fields. Phase Care’s enrollment service receives a payment reference, not the card number or security code. Enrollment uses a necessary, short-lived session cookie. Google Tag Manager and Google Analytics are not loaded on the membership enrollment page.
Website and analytics information
The site may collect routine technical information such as browser type, device type, pages visited, approximate location, referral source, and interactions with calls to action. Phase Care uses Google Tag Manager and Google Analytics to understand site performance and improve the website. The site’s custom analytics events do not intentionally send form contents, booking details, or health information.
