Know what is separate
Outside services are not hidden inside the price.
Membership covers clinical care delivered by Phase Care. The following remain separate patient costs.
Laboratory testing
Lab fees are separate, even when Dr. Sheikh orders or interprets the results.
Imaging & procedures
Radiology, procedures, and services performed by outside facilities are separate.
Specialty & hospital care
Specialists, emergency departments, hospitals, and other clinicians bill separately.
Insurance
You can join with any insurance—or without it.
Phase Care does not submit insurance claims. Membership is available regardless of the insurance you carry.
Direct primary care is not health insurance. Many patients keep insurance or another health plan for medications, labs, imaging, specialists, hospital care, and emergencies outside the membership.

Before you enroll
Is there an enrollment fee, and how do cancellations work?
There is no enrollment fee. Membership is $250 per month. Cancellation arrangements are customized to each member, taking changes in circumstances into account. Contact Phase Care to discuss your membership.
Are office visits really included?
Primary care visits provided by Phase Care are included in membership without a separate office-visit copay. Services and supplies from outside organizations are separate.
Does the membership include weight-loss medication or testosterone?

Can I meet Dr. Sheikh before joining?
Yes. If you are not ready to enroll, you can schedule a complimentary first visit to meet Dr. Sheikh and discuss whether Phase Care fits your needs.
